Cover Sheets for List of Creditors and the Mailing  Matrix in the Case of a Non-Individual Debtor (a Corporation, Partnership, Unincorporated Association, etc.)

Whenever a debtor that is not an individual (for example, when the debtor is a corporation, a partnership, or an unincorporated association), files a List of Creditors and Mailing Matrix, it should adapt the Mailing Matrix Coversheet - Individual Debtor(s) Case that is posted on this website for use by a debtor who is an individual, and utilize that adapted cover sheet, with the name and position of the entity signing the Amendment on behalf of the debtor being typewritten underneath the signature line.
